DLA form help filling out part one Q1 – 13

Getting DLA i have written about before but i have never did notes on helping you get your DLA. I will add to this the information i give is my own opinion and is not the view of the department of work and pensions and i could be wrong. Then i will add to that i did get my DLA no appeals either. your form might be slightly different to mine as well if so you could always scan it at the local library and email it to me or send it to me.

right filling the form out first of all your going to need a black pen blue pens are useless and it does ask for a black ink pen your also going to have to be organised here or its going to take forever to fill out.

find your national insurance number you will most likely have a card some where if you have lost it go to your local job centre im sure they will be able to find it for you.

if you have any letter’s that will support your claim it would be really useful to find them as well maybe take a photo copy you could also goto your doctor he has most of your records.

 

1 surname of family name this is quite simple whats your second name like me im Kevin john blumer so my family name is Blumer if it was say my borther Joe Mark Blumer for example then again the surname is Blumer

2 all other names in full  ok again we would just be putting Kevin john here simple

3 title well this is quite easy to if you are over 16 your mr or miss if your married its mrs for a woman

4 your national insurance number remember me saying you would need it this is it

5 date of birth if you don’t know this and some people don’t  ask your mom or dad or find your birth certificate another way of putting it is when where you born

6 sex easy enuff are you a male in case m or female f

7 the place you live your address have a look for a bank statement it will have your postcode on as well i can never remember my address and postcode i’m useless at it.

8 daytime phone number if you don’t have a phone use your mobile number it’s very useful for them to have your phone number to contact you if something is wrong or they need to make sure something is right if you don’t have a mobile use a friend you trust maybe your parents even if you fail to put a number down and something is wrong it’s going to slow your application down loads

9 whats your nationality from the readers on this site i will take it you are british or from the uk if you are not i cant help you no further  because it gets quite complicated.

10 next the question asks if you usually live in the uk again i cant help you if you don’t  but if you do live in great Britton anser yes

have you been out the uk for more than 13 weeks most likely not the rules change if you have there just trying to make sure that you live in the uk so long as you havent been out for more than 14 weeks you can anser no   if you anser yes you will have to explain why maybe you were on an extended holiday break

10 your accommodation  well this is quite simple but could be complicated you might live with your parents or you could live in supported accommodation all these things they need to know and will help you or you could be living in university accommodation or by yourself.

right they are the basics and probably the easy part of the form but we do move on to questions that can be quite personal next.

where is there a toilet in your home upstairs or downstairs or it could be shared and along the corridor and up the stairs just put it as it is.

where do you sleep in your home upstairs downstairs i would imagine these questions are for part of the mobility component if you sleep on the setee in the living room say so never lie to make your form look better. i spent a year living in a chair and no bed so i would have had to put that.

if you have someone else filling out your form make sure they tick the right boxes there are quite a few questions just get the person to tick the one that applies to you if someone else is filling out the form they have to give their details as well so be prepared once you start filling this form out it may well be worth reading this post before starting.

about your illnesses section

right for this part this is where your doctors notes and letters come in very handy you have to be very exact to what you put here you can not just self diagnose because you will be unable to back it up it has to be certified doctor or medical health professional or any other body that can explain you condition or illness for learning disability for example you have social workers and psychologists if you have a bad heart you are going to have letters off cardiology these are really going to help with out these your going to have no way to back yourself up there for its here say.

if you take tablets get your doctor to print out what you take and how often you can include what the side affects are for treatments you might have DBT if you are a person that can’t cope with the world ill do you a little example

in other words when were you diagnosed as been an alcoholic  we will say 9 years

what medicines or treatments do you receive

Diazepam 45 mg daily and counselling 2 per week

your own circumstances are going to be different but i think it helps people to see different examples  gives a much better idea of what to put.

Letter from psychotherapist 2

“Mr Blumer presents with significant borderline and narcissistic personality disturbance but not with formal mental illness. for that reason he may fall beneath the current clinical threshold of current community mental team involvement. this may change in the future with the planned introduction within the mental health trust of specialist personality disorder nurses. He has indicated that his care co-ordination was helpful to him in Bournemouth”

ok so if i don’t have mental illness what do i have a mucked up head could be going against the original report. From what i can tell and understand i go on like a bpd but i am not does not make sense much to me but there you go. Mental health teams in the north of england will not help people with borderline they say yer we have this and we have specialist people etc and build you up and you get nothing. At the moment i have had no involvement with any of the services i have been refered again to another service but i don’t hold any hope out for them either . i have to wait another month of so been waiting since my last appointment with Ian in december i use to have a person called Justina pang and she was very help full in Bournemouth. I have currently been refered to the primary mental health care trust i don’t know what it’s about i don’t know if it will help i will just have to wait and see and survive in the meantime.

Letter from psychotherapist 1

Part One

“As indicated in my recent letter to you Mr Blumer has been discharged from our service. I am sending a more detailed report in hope that this will be helpful to the services involved in supporting him”

Ok first part of the letter really simple for anyone to understand basic I am no longer going to Ian. The next part the services that are helping me none as such Byker bridge were supposed to be here in November. There is the housing support from Newcastle city council witch are really helpful and soon the primary health care don’t hold much hope out with them either and I live in kind of supported accommodation where you have the chance to move on but they are not good with mental health when you are in crisis its easier just to put yourself in hospital. I will add that camerons 450 million is not helping people like me because I can not have CPN up north because you have to either be a threat or really unsafe to yourself.

 

Does smoking give you confidence


Smoking and confidence 

No im not advertising smoking im asking a question here there is a big thing at the moment to get people to stop smoking for the good of their health right ok. I have stopped and it makes life not worth living and also i can no longer talk to people and i no longer feel like me i lose all of my confidence. So maybe im no expert smoking gives you a bit of a high it allows you to do things you could not other do so i wonder if there is a way to combat that for the good of my health what is it that creates my brain to be able to be confident when i smoke take it a way and i shake quite a bit as well. What causes this with me is it a part of smoking that makes you high i really don’t know the anser i put this question out to my readers of this blog.

My finale report off the therapist

right its too late tonight but i will write it out over the next week its quite long eight pages it has all sorts i want to take it apart and make something of it. and its going to be a long wirte upand probably going to take a couple of weeks to get it up. I think its perfect for this site and it will make good reading for anyone who knows my story  and it will be good for people who dont know me.i dont write here as much these days but not becuase i dont want to im in the process of moving and eventualy work for myself  and some of that is stating to really take shape so if you want to read a discharge from a therapist after nealy a year 

stay tunened what i have allways said i am clear and transparent most everything is on here loads i havent added just yet but loads to come

BPD blogs and Mental Health blogs submit

Its been a while since i have posted on here and i have had a lot of problems over at kbos2 just teething problems really this blog is extremely popular at the moment not sure why but there you go. I’m currently updating two pages at the moment on here one is the support page and my next page mental health support if you want to be in the support page and you have a bpd blog please do comment and i will add it on i cant promise loads of traffic but your blog will be seen you must have been writing your blog for over 3 month as well. Stop and review the cuts to benefits

As a lot of people will be aware the government is trying to change dla and esa these reforms as they call them are going to hit a lot of people and probably the worst affected would be the worst off. I heard a story the other day about a bloke who was 26 quite young could not eat by himself cut not talk properly and had care basically 24 hours a day and his money was stopped why because he could maybe hand leaflets out so there for he was able to do something he lost his money and all his support had to rally fight to get it back for him that side of the cuts we dont see would you want that bloke doing leaflets for your company. What kind of life does he have its one of the most shocking story i have heard. Facebook BPD support groups

I will start this off with please share and share alike. Right when i am in trouble or need support i have four channels and im getting better i will tell you them i have Facebook closed secret and open groups a forum for things that i would rather be handled not on Facebook and then i have a blog then finally i have my own blog or my friends im going to add the Facebook bpd support groups here so people can join if you have any more please do add them here and i can then put you in the admin and support groups.

My own little support group on Facebook we are small but also very supportive of one another. I also use this as a testing ground for adding people to other support groups so i can try to judge whether you will be better in a large group or a small group if you’re an admin for any support groups on Facebook please add-on this post and join the group and i can then get you in the admins in support

Can we trust what we see

We see colour  and shapes everyday but can we trust what we see and what we hear. If we are normal and don’t have problems with mental health we have no problems with that. But how about we see things that are not real i think everyone has seen things that are not there. non prescription tablets can cause this and so can drink. I think it all comes down to trust and what we have learned i have a saying for it if it don’t seem real it probably isn’t what have you seen in your life time that’s not real.

Why cant i get DLA

Good question three people now have asked me this DLA is not the easiest thing to get believe me. But there are some thing that you can do to help yourself don’t fill the forms out yourself take them either to the job centre if they wont deal with them there is citizens advice they will always give you a hand. Back everything up with paper if you have letters from doctor’s  psychologists anybody like that include them this will help them a great deal and make things a lot easier for you too still its no walk in the park theres letter from hospitals as well they can sometimes be useful. And if that all fails there is also appealing but i would go down to CAB if you are having problems. they’re a charity and they are there to help people.
